What is this book called?

I read a book a couple years back, and it was in my opinion one of the greatest books ever. In the title was something about "plague" because the story had to do with the black plague. This book took place far into the future in a utopian society where everyone was to cover their faces, and never show any creativity or lack of conformity. Everyone usually referred to each other with numbers, not names. And when one person is discovered to be a free thinker, they take him and put him in the shoes of a Jew during the Black Death, and tried to associate music with death so that he doesn't want to think creatively anymore as to "cure" him. All I remember about the title is it contains or is "Plague". Please help me! It would mean so much!
